U.S. Provisioning Forms
The following forms can be used to submit your provisioning requests to Vibes.
COMPLIANCE: It's important to keep in mind that the CTIA & Carrier Compliance Requirements apply to all SMS & RCS programs, regardless of delivery method. So, please ensure your program includes all of the carrier required compliance disclosures and will not send any prohibited content. If a brand is unknown, please also ensure that thorough brand vetting is completed before requesting enablement of their code.
BRAND VETTING: In order to lease a U.S. shortcode, 10 DLC, or launch an Agent in production, the carriers will require that the brand pass vetting. U.S. shortcodes also have an annual re-vet process.
If Vibes will be leasing the brand's shortcode in our shortcode registry account, provisioning a 10 DLC code on your customer's behalf or requesting that the carriers launch an Agent on their network, please provide the brand's information to Vibes using this brand vetting intake form. If you or your customer will be leasing the shortcode in your account, that account owner will need to complete the brand vetting steps when leasing the code.
SHORTCODE PROVISIONING FORMS
Vibes Blank Shortcode Program Brief - This document will be used to submit your U.S. shortcode provisioning request to the carriers. This document should include a description of who the code's content provider is, what kind of content will be sent to participants, where the program will be promoted, the MT content for the opt-in message, sample alerts, help and stop. Note, web opt-in programs must include a screen shot or mock up of the web call-to-action and the URL showing where it will be located once it's launched in market.
Vibes Shortcode Program Brief with Instructions - This version of the shortcode program brief template includes instructions and compliance requirement notes.
T-Mobile's Special Business Review Form - This document is only required if we'll be requesting T-Mobile's approval for a campaign that does not meet the carrier's normal compliance requirements. Programs that will be asking for an exception to the carrier's compliance requirements typically require an additional 1-3 weeks in campaign review.

TOLL-FREE PROVISIONING FORMS
TFN Verified Sender Request Form - This document will be used to submit your toll free code's provisioning request to the carriers. This document should include a description of who the code's content provider is, what kind of content will be sent to participants, where the program will be promoted, the MT content for the opt-in message, sample alerts, help and stop. Note, web opt-in programs must include a screen shot, mock up, or URL showing the web call-to-action.
TFN Verified Sender Request Form with Instructions - This version of the toll-free sender request document includes instructions and compliance requirement notes.
Vibes will review the program brief/sender request form prior to submitting to the carriers and will notify you if any updates are needed in order to move forward. Once submitted to the carriers, a code's provisioning progress can be tracked through the weekly provisioning reports that are provided by your Vibes Account Manager.
